Biostimulatory fillers work differently from conventional dermal fillers. The two main types used in aesthetic medicine are PLLA (poly-L-lactic acid) and PDLLA (poly-D,L-lactic acid). Both are biocompatible, biodegradable polylactic acid compounds that stimulate collagen through the same core mechanism, with some differences in their molecular composition and degradation profile.
Results develop gradually over several weeks as new collagen builds, rather than appearing immediately. How It Works
Collagen induction
Polylactic acid microspheres are injected into the deep dermis or subcutaneous layer. The microspheres act as a scaffold, triggering fibroblasts to produce new collagen around them, gradually rebuilding the skin's structural support from within.
PLLA vs PDLLA
PLLA (poly-L-lactic acid) and PDLLA (poly-D,L-lactic acid) are both polylactic acid biostimulators that achieve collagen stimulation through the same biological mechanism. PLLA is composed of a single molecular isomer and has a long track record in aesthetic medicine. PDLLA contains a combination of isomers, which affects its molecular configuration and degradation profile. Your doctor will advise on which formulation is most appropriate for you.
Gradual, natural progression
Unlike HA fillers, results develop progressively over 4 to 8 weeks as collagen production increases. The full effect is typically visible 2 to 3 months after the final session.

What to Expect
The Procedure
Sessions take 30 to 60 minutes. Topical numbing cream is applied beforehand. Injections are made at specific anatomical points using a fine needle or cannula. Mild pressure at each injection site is normal.
Downtime
Mild swelling, tenderness, and redness for 1 to 3 days. Small nodules may be felt under the skin as the product integrates; these typically resolve within a few weeks. Avoid strenuous activity for 24 hours.
